I don't find it complicated. I would want someone to respect my wishes. Even if I intended to harm myself. I would want someone to respect my dignity and autonomy as a human being.
That you describe your sister's preferences as nuts shows a level of not just disagreement but a decided disrespect for her as a person, one who because she does not see things your way, does not merit your respect but only condescension.
And maybe this is what you feel towards the apologist and philosopher as well as others who simply don't see things your way.
